Fish can be pre-ordered for both spring and fall (same form is used) through our online ordering website or by filling out the PDF below and returning to PCSWCD the Friday before fish delivery/pick up is scheduled. Paper copies are available for pick up at the USDA Service Center (1209 Bear Lane, Monticello IL) as well. If you would like a form mailed to you, please call (217) 762-2571 ext. 0947
